You should not have to travel simply to receive routine care that is readily available close to home.

But there are situations where a focused cervical dysplasia evaluation can add something different.

Many of the women who contact us are not simply looking for someone to perform a procedure.

Cervical dysplasia is not simply something we occasionally encounter as part of a larger practice. When preserving cervical tissue matters
For women who hope to become pregnant in the future—or who have already undergone one or more cervical procedures—the amount of cervical tissue removed can matter. Preventing cervical cancer always comes first. But when more than one clinically reasonable option exists, previous treatment, reproductive plans, and preservation of healthy cervical tissue belong in the discussion.
SELECTED NONSURGICAL TREATMENT
Expertise in emerging treatment approaches
For carefully selected patients, we may discuss nonsurgical treatment with topical imiquimod. Imiquimod is not FDA-approved for cervical dysplasia and is not appropriate for every patient. Established excisional treatment remains more effective overall. Our approach is to explain both the published evidence and its limitations so that an off-label treatment is considered only when it makes sense for the individual situation.

Start Before You Travel

We do not want you to travel hundreds of miles only to discover that the visit could not accomplish what you hoped.

Whenever possible, the process begins with the records you already have.

That may include:

  • Pap or cytology reports
  • HPV testing and HPV genotype
  • Colposcopy reports
  • Cervical biopsy pathology
  • Endocervical sampling results
  • Previous LEEP or cone pathology
  • Margin status
  • Previous cervical treatment records
  • Relevant screening history

Reviewing this information helps us understand why you are seeking another evaluation and what may need to happen when you arrive.

What Can We Accomplish During Your Visit?

Every patient is different, and we cannot promise that an entire diagnosis and treatment process can be completed in one trip.

But thoughtful planning before you arrive can make the visit substantially more useful.

Depending on your situation, your Chattanooga visit may include:

We review your previous history and explain what your results mean, what your estimated risk is, and why particular recommendations may or may not apply.

In-person cervical examination

Sometimes the anatomy of the cervix is itself important to deciding what options are safe.

Colposcopy

If colposcopy is appropriate, this can often be incorporated into the in-person evaluation.

Cervical biopsy or endocervical sampling

When additional tissue is needed to safely determine the next step, specimens can be obtained and submitted to pathology.

Treatment planning

We can discuss surveillance, excisional treatment, ablative approaches when appropriate, selected nonsurgical treatment, or referral.

A clear follow-up plan

Even when treatment cannot happen during the same trip, you should leave understanding what happens next.

Why an In-Person Visit Can Matter

Records can tell us a great deal.

They cannot tell us everything.

When considering treatment options for cervical dysplasia, direct examination may help determine:

  • Whether the entire transformation zone can be seen
  • Whether the upper limit of a lesion is visible
  • Whether disease appears to extend into the endocervical canal
  • Whether an ablative or other cervix-preserving approach could be considered safely
  • Whether additional biopsy is needed
  • Whether the clinical examination agrees with the previous records

This is one reason we do not want complex treatment decisions to become simply a remote exchange of pathology reports.

Sometimes you need someone to actually examine the cervix.

 

What if the answer is still LEEP?

Then we will tell you.

Traveling to see us does not guarantee that we will recommend something different from your current physician.

Sometimes reviewing the history and examining the cervix confirms that an excisional procedure is exactly the right next step.

That is still a useful answer.

A good second opinion is not one that necessarily gives you a different recommendation.

It is one that helps you understand why the recommendation is right.

Do I need a referral?
No. You may contact us directly for a cervical dysplasia consultation or second opinion.
Should I send my records before I book travel?
Yes, whenever possible. The more complete the information we have beforehand, the better we can understand what type of in-person evaluation may be useful.
Can you tell me before I come whether I qualify for imiquimod?
We may be able to determine that imiquimod is clearly not appropriate based on your existing records. Determining that it is appropriate generally requires a more complete clinical evaluation and may require direct examination of the cervix.
Can you guarantee everything will be completed in one trip?
No. Sometimes the safest next step depends on new biopsy results or other information that is not available until after your visit. We will try to make the trip as efficient as we reasonably can without compromising the clinical decision.
Can my spouse, partner, or family member come with me?
Yes. If having someone with you makes the visit easier, you are welcome to bring a support person.
What if I decide not to have treatment after the consultation?
That is completely acceptable. A consultation is not a commitment to treatment. Sometimes the outcome is surveillance. Sometimes it is returning to your local physician with a clearer understanding of the plan. Sometimes it is proceeding with treatment here.
Do you treat cervical cancer?
No. If your records or examination raise concern for invasive cervical cancer, we will recommend evaluation by an appropriate gynecologic oncologist.
What should I bring with me to my appointment?
Bring any cervical records you have that we have not already received, along with a current medication list and any questions you want to make sure we address. If you are traveling a significant distance, it can also be helpful to bring a support person if that would make the visit easier for you. If we have already reviewed your records before you arrive, you do not need to recreate the entire history from memory. The goal is to spend the visit understanding your situation and deciding what happens next.
