Hitchcock Family Medicine Covid-19 Testing

Hitchcock Family Medicine is happy to partner with Phase2 Laboratories to provide Saliva PCR Testing services in our offices. 


Testing is available by appointment only at our Eastgate office Monday through Friday from 10:00 am to 2:00 pm.


You do not have to be a member of Hitchcock Family Medicine to schedule COVID-19 testing through us.

Cost is $130 for non-members and due at time of registration. Inquire about bulk discounts for businesses. No Walk-Ins! This is by appointment only.

We do not bill insurance, but you will receive an invoice by email with the proper coding that you can submit to your insurance to try to get reimbursed.


You may use an HSA or FSA to pay for this test, simply enter that card when prompted for your credit card information.

How to Register

  • Please register here to schedule your test. (This is not for membership to Hitchcock Family Medicine, only for COVID testing)

  • Enter all information requested on the registration form, including location (Eastgate or Hixson) and payment information. You do not need to select a physician, as you are not registering for membership.

  • You must register every person who would like to be tested.

  • Once you submit the registration, you will receive a confirmation on your screen and a confirmation email (please check your spam folder if you don’t see it right away.)

  • Once we receive your registration, we will email you a link to the schedule so you can book your appointment. (Please check your spam folder if you don’t see it right away.)
  • Click the link to book your appointment. You must book an appointment for each person that you registered for testing.
  • Your card will be charged when you book your appointment.

  • Do not put anything in your mouth for thirty minutes before your test time. This means nothing to eat, drink, smoke, or chew for 30 minutes before you provide your saliva sample.

  • Please come to your selected location at your scheduled appointment time.

  • Remain in your car and keep your mask on your face, covering your mouth and nose.

  • A staff member will bring you your test kit and answer any questions you may have. 

  • Please remain in your car to complete the lab-specific registration and provide your saliva sample. (Directions for the lab registration are in your test kit)

  • After you have submitted your lab form and provided your specimen, please ensure your tube and bag are labeled according to the directions provided and place your specimen in the labeled specimen return box.

  • You are free to go! The lab will text you a link to your results within 24 to 48 hours.

  • If you do not receive your test results from the lab within 48 hours, please email us at COVID19@Hitchcock.MD.

  • If you would like to test again in the future with us, you will not need to re-register here, but can simply use the scheduling link to book your appointment. We will charge the same payment source entered initially, so please email us at COVID19@Hitchcock.MD if you need to change this PRIOR to booking your new appointment.
